Southwest Dietetic Group, Inc. accepts provider referrals for Medical Nutrition Therapy. Referral forms are available for your convenience.
As providers of Medical Nutrition Therapy, our dietitians strive to be an integral part of a coordinated care team in the outpatient setting. We believe in establishing open lines of communication between all providers whenever possible. To enhance your practice we provide the following services:
Nutrition Consult Summaries
After each visit, our dietitians will send a summary including the patient’s progress, recommendations and current goals. Nutrition Consult Summaries are provided to all authorized members of the care team.
Patient Nutrition Care Plan
Patients are provided with a nutrition care plan at their initial assessment to outline personalized nutrition goals. Follow-up appointments will be used to evaluate progress and assist with barriers to change.
Location Flexibility for Service
Dietitians are available to come to your facility to provide nutrition counseling if medically necessary for the patient.
